A clean, secure registration form — files are saved into your Drive folder and details into the Registrations sheet.

Fields: shop name, owner name, mobile, email, city, UPI, plan, comments & optional document upload.

How it works

  1. Fill form and optionally attach document (GST/ID/license).
  2. Client converts file to base64 and sends JSON to your Apps Script.
  3. Apps Script saves the file to your Drive folder and appends a row in your sheet with file URL.

Keep file size under 10 MB to avoid script timeouts and upload limits.
